The Results of Limb Salvage Surgery with Tumor Prosthesis -An Analysis of Complications and Functional Results-

Abstract
We reviewed the treatment results of the patients who underwent limb salvage surgery with prosthetic reconstruction due to primary bone tumor, and evaluated the complications related to the prosthesis and the functional outcomes. Between 1985 and 1995, 102 patients underwent endoprosthetic reconstruction. Their average age was 31 years with a range from 10 to 66 years and the average follow up of all patients was 54 months. The diagnoses were osteosarcoma in 57 patients, chondrosarcoma in 17, aggressive giant cell tumor in 16, parosteal osteosarcoma in 7, and one case each of periosteal osteogarcoma, Ewing¢¥s sarcoma, lymphoma, malignant fibrous histiocytoma and fibrosarcoma. For reconstruction, we used Kotz modular endoprostheses in 45 patients and custommade prostheses in 57. Seven extendable prostheses and seven distal tibial prostheses were included in the 57 custom-made prostheses. There were 38 complications in 28 patients. : local wound infection developed in 12 patients, wound problems in 7, local recurrence in 5, fracture in 3, implant fractures in 2, aseptic loosening in 3, dislocation in 2, and one case each of peroneal nerve palsy, myositis ossificans, metal irritation and polyethylene wear. Revision was performed in 8 patients due to 3 aseptic loosening,2 implant fracture, 1
infection, 1 metal irritation and 1 polyethylene wear. Four amputations were done due to local recurrence in 3 patients and deep infection in 1, but most complications were salvageable without loss of limb. The survival rate of the prosthesis was 85 per cent at 5 years and 72 per cent at 7.5 years, while the clinical results were excellent or good in 58.1 percent.
